Registration Forms

There will be three new forms required to race this year, two per rider.

A one time Association Membership Application, Minor Waiver of Liability and Indemnity form.

A one time Association Membership Application, Adult Waiver of Liability and Indemnity form.

Adult_ Association_ Waiver_Form.pdf

These forms are to be filled out once and the $30.00 fee paid, if they are filled out at the track it will be notarized for the minors and witnessed by a Lion¢â¡Ás member for the adults.

The last new form is the Rider Class Registration form; this form will be filled out at each event.
